Non receipt of redemption proceeds

On October, 10 2008, I had personally deposited the UTI- Monthly Income Plan- 1998- Cumulative Growth (Regd. Folio 4001830035000 Membership No. 400-98-183-0035000, Distintictive Nos. N 446487795- N 446490294) certificate (original) in the name of my father- late Shri Madan Gopal Mahendra along with the duly attested death certificate of my father- late Shri Madan Gopal Mahendra with your Lucknow Branch (copy of acknowledgement enclosed) for remitting the redemption proceeds to me at my address given below. However, despite a lapse of about six months as also reminder dated November 20, 2008 to UTI Mutual Fund, I have heard nothing in the matter from them. I , therefore, request you to please look into the matter and instruct UTI Mutual Fund to remit the maturity proceeds
